The questionnaire consists of 20 key topics besides general data on Demographics and Vital Signs.
Each question will focus on the aspects critical to pregnancy and suggest course correction where applicable.
A report with your answers will be generated at the end and will indicate your risk score by flagging areas that you need to focus on or reach out to a physician prior to planning a pregnancy.
The report can be downloaded or emailed to your registered email ID.
This questionnaire can be taken multiple times over the course of your pregnancy planning period to reflect your status and progress.
